"Improves circulation" is the explanation attached to scalp massage in roughly every hair-growth roundup that mentions it, usually right next to a stock photo of hands on a scalp. It's a plausible-sounding claim that happens to not be what the actual research measured.
The studies that exist point somewhere more specific: not blood flow as the primary driver, but mechanical force. Stretching the tissue around the follicle, directly, in a way that changes what the cells there are doing.
The mechanism: stretch, not circulation
The clearest evidence comes from a study combining a human trial with cell-level analysis. Nine men received four minutes of standardized scalp massage daily for 24 weeks. Hair thickness increased measurably over that period. To understand why, the researchers modeled the mechanical effect of the massage on the scalp's subcutaneous tissue and found it produced measurable physical displacement and stress. They then cultured human dermal papilla cells (the cluster of specialized cells at the base of the follicle that regulates hair growth) under a repeated stretching cycle in the lab, and found significant gene expression changes, including increases in several genes associated with the hair growth cycle.[1]
This lines up with a broader mechanism identified in regenerative research: mechanical stretch, applied to skin, has been shown to activate hair follicle stem cells and drive regeneration through a specific signaling sequence. In one study using mice, stretching recruited immune cells called macrophages to the area, which then released growth factors that activated dormant follicle stem cells and triggered new hair growth, a process the researchers found only occurred within a specific window of strain intensity and duration.[2] Scalp massage plausibly works on the same principle: mechanical load on tissue as a trigger for follicle activity, not warmth or blood delivery as the mechanism.
What this looks like outside a research study
A survey study asked people self-identifying with androgenic alopecia (the most common pattern of progressive hair thinning) to try twice-daily, 20-minute standardized scalp massages and report back months later. Adherent participants averaged about seven months of massaging, and a majority reported some degree of hair loss stabilization or regrowth, with the effect trending stronger the more cumulative time people put in.[3]
This detail matters more than the headline result: this was a self-reported survey, not a controlled trial with an objective measurement arm, and it can't rule out expectation effects or people who stuck with a demanding routine because they were already seeing something they liked. It's suggestive, consistent with the mechanical-stretch mechanism above, and not proof on its own. The cell-level and controlled-trial evidence is what actually supports the mechanism; the survey is what supports "people who do this for months tend to say it helped."
The consistent thread across all three studies is duration and consistency, not intensity. Four minutes daily for 24 weeks, or 20 minutes twice daily sustained for months, not one aggressive session before a hair appointment.
How this differs from other 'growth' claims
Scalp massage is often mentioned alongside topical ingredients marketed for growth, and it's worth being precise about why they're not interchangeable claims resting on the same evidence. Rice bran and rice water, for instance, have research behind a completely different mechanism: bioactive compounds acting at the follicular level, including inhibiting an enzyme involved in androgenic hair thinning and upregulating growth-signaling factors.[4] That's a biochemical mechanism, ingredients interacting with follicle biology from the inside. Scalp massage's mechanism is mechanical, physical force on tissue that follicle cells respond to structurally. Both can plausibly contribute to hair health through entirely separate pathways, and neither substitutes for the other. Someone using rice water rinses and getting no benefit from them isn't a candidate to expect the same result from scalp massage instead; they're different levers.

What scalp massage doesn't do
None of this evidence extends to reversing established, advanced hair loss, or to treating conditions with a distinct medical cause. The self-assessment survey specifically excluded people already on prescription treatments and looked at people with mild to moderate self-perceived thinning; it wasn't tested as a standalone treatment for diagnosed, advanced androgenic alopecia.
It's also not a fast intervention. Every study cited here ran for months, not weeks. A single relaxing scalp massage feels good and may genuinely help distribute natural scalp oils, but it isn't the dose the growth research is describing.

Scalp massage is a low-cost, low-risk practice with a genuine, if modest, mechanical rationale behind it: several minutes a day, sustained over months, appears to create enough physical stretch at the follicle to measurably influence the growth cycle.
